Friendly Notice for Applicants Under 18 Years Old



For applicants under 18 years old, besides the basic and supporting documents, please prepare the additional documents as below:
1. A photocopy of the Birth Certificate.
2. A photocopy of Parents' valid ID card or passport information page.
3. If the name of the applicant or his/her parents' current names differ from those on the Birth Certificate, a photocopy of the first name or surname change is required.
4. If the applicants do not travel to China together with their parents, a Letter of Consent from a mother/father who does not go to China is required.Parents can request the Letter of Consent from the District Office or write it by themselves on a photocopy of Parents' valid ID card or passport.The details should contain the following:
- Parents' name.
- Be a guardian of the Child.
- What is the purpose of visiting China for the child?
- Who is allowed to travel with the child?
- What is the expected arrival date in China?
- After that parents sign their names.
- Please write the letter in English or Chinese.
5. On the application form, 'Applicant's signature' must be signed by both Parents.
